McKenzie Exercises for Neck Pain

McKenzie exercises are an effective therapeutic program designed to help relieve neck pain associated with muscle strain, prolonged sitting, or poor head posture.

The exercises aim to reduce pressure on the cervical spine, improve neck mobility and alignment, and gradually ease stiffness and muscle tension.

Guidelines Before Performing the Exercises

  • Perform the exercises slowly.
  • Move within your comfort range.
  • Stop immediately if you experience sharp pain or dizziness.
  • Maintain proper sitting posture.
  • Breathe normally.
  • Do not apply excessive pressure to the neck or head.

Remember: Do not perform neck exercises in the following cases:

  • Severe pain or a recent injury.
  • Numbness or weakness in the arms.
  • Persistent dizziness or loss of balance.


1. Chin Tuck Exercise

To strengthen the neck muscles and improve head alignment.

How to Perform the Exercise:

  1. Sit upright with your shoulders relaxed and look straight ahead.
  2. Gently pull your chin inward (backward) without tilting your head, as if making a double chin.
  3. Hold the position for 5–10 seconds.
  4. Repeat the exercise 10 times, three times daily.

2. Neck Extension Exercise

To improve neck mobility and reduce stiffness.

How to Perform the Exercise:

  1. Sit upright with your shoulders relaxed and look straight ahead.
  2. Gently pull your chin slightly inward.
  3. Slowly tilt your head backward as far as comfortably possible.
  4. Lift your face upward as if looking at the ceiling.
  5. Do not let your head drop forward quickly; slowly return to the chin-tucked position.
  6. Repeat the exercise 10 times, three times daily

3. Lateral Neck Stretch

To improve flexibility of the side neck muscles and reduce tension.

How to Perform the Exercise:

  1. Sit upright with your shoulders relaxed and look straight ahead.
  2. Place your left hand over your right ear.
  3. Slowly tilt your head to the left (toward your shoulder).
  4. Gently use your hand to pull your head to the side for a deeper stretch, without applying excessive pressure.
  5. Hold the position for 3–5 seconds.
  6. Repeat the exercise using the same steps on the other side. 

4. Neck Rotation

To improve neck mobility and reduce stiffness.

How to Perform the Exercise:

  1. Sit upright with your shoulders relaxed and look straight ahead.
  2. Slowly turn your head to the right.
  3. Hold the position for 3–5 seconds, feeling a gentle stretch.
  4. Slowly return to the starting position.
  5. Repeat the exercise on the left side.
  6. Repeat the exercise 10 times, three times daily.

5. Forward Neck Flexion

To stretch the muscles at the back of the neck and reduce tension.

How to Perform the Exercise:

  1. Slowly bend your head toward your chest.
  2. Place your hands behind your head with your fingers interlaced, keeping your elbows in a comfortable position.
  3. Gently use the weight of your hands to increase the stretch without applying excessive pressure.
  4. Hold the position for 3–5 seconds.
  5. Repeat the exercise 10 times, three times daily.

6. Lateral Neck Stretch

To improve flexibility of the side neck muscles and reduce tension.

How to Perform the Exercise:

  1. Place your left hand over your right ear.
  2. Slowly tilt your head to the left (toward your shoulder).
  3. Gently use your hand to pull your head to the side for a deeper stretch, without applying excessive pressure.
  4. Hold the position for 3–5 seconds.
  5. Repeat the exercise using the same steps on the other side.

7. Arm Raise with Upward Gaze

To improve posture and reduce tension in the neck and shoulders.

How to Perform the Exercise:

  1. Sit or stand upright with your back straight and your shoulders relaxed.
  2. Keep your head in a neutral position and look straight ahead.
  3. Interlace your fingers in front of your body.
  4. Turn your palms outward, then slowly raise your hands above your head.
  5. As you raise your hands, gradually lift your head and look toward your hands.
  6. Hold the position for 10–15 seconds. You will feel a gentle stretch in your neck and shoulders.
  7. Slowly lower your hands and return your head to the starting position.
  8. Repeat the exercise 3–5 times, three times daily

8. Front Neck Resistance

To strengthen the front neck muscles and improve stability.

How to Perform the Exercise:

  1. Sit upright on a chair with your back straight and your shoulders relaxed.
  2. Keep your head in a neutral position and look straight ahead.
  3. Place the palm of your hand on your forehead (front of the head).
  4. Gently press your hand against your forehead toward the back.
  5. Resist the pressure with your head by pushing forward, keeping your head stable without moving.
  6. Hold the position. You will feel gentle tension in your neck muscles.
  7. Relax and return to the starting position.
  8. Repeat the exercise 10 times, three times daily.

9. Shoulder Shrugs

To reduce muscle tension in the shoulders and neck.

How to Perform the Exercise:

  1. Sit upright on a chair with your back straight and your shoulders relaxed.
  2. Keep your head in a neutral position and look straight ahead.
  3. Slowly raise your shoulders toward your ears as high as possible.
  4. Inhale as you raise your shoulders.
  5. Hold the position for 5–10 seconds.
  6. Slowly lower your shoulders while exhaling.
  7. Return to the starting position.
  8. Repeat the exercise 10 times, three times daily.
